To ensure the safety of your pets and the quality of care provided, all required forms must be fully completed and signed by both the pet owner and Kota’s Kamp before any services begin.

At a minimum, the Service Agreement must be completed and signed before any services can be scheduled or fulfilled. Services will not be provided without this agreement on file.

Forms may be signed electronically online or printed and signed physically. All new clients will receive a physical copy of the forms during their meet-and-greet.
